Join this Special Opportunity for Veterans in Your Care
HCA urges all home care providers, managed long term care plans, hospices, FI/Consumer Directed Personal Assistance Plans and home care waiver programs caring for veterans to join in this special effort to be part of the “Virtual Community for Veterans” Pilot. This special support service is made possible for your veterans through a generous grant to HCA from the New York Health Foundation. The pilot supports homebound and medically needy veterans with daily peer socialization, self-care and wellness guidance, professional assistance, and an array of additional services specific to veterans’ needs, interests, quality of life enrichment, and enjoyment.

Is there a fee to sign up?
Thanks for the support from the New York Health Foundation, it is completely FREE to sign up and join unlimited programs!
I’ve signed up, what is the next step?
Join one of our New Member Orientations happening every Wednesday & Friday at 2 pm. Our VSC Technical Support Specialist will show you how to log on to the VSC, navigate the platform, find programs that interest you, use the video chat, and how to reach out for technical help when you need us.
